New Patient Paperwork

When you book your first appointment you will have the option of filling out your paperwork online prior to your first visit or arriving early to fill out a hard copy of our new patient forms. Please share your preferences with our front desk staff. 
We recognize that products, recommendations, and anecdotes we discuss and share have been used for generations by Indigenous Peoples. Their study of medicines and formulas have paved the way for patented medicines and the studies there of though they are not yet, recognized by the FDA. To uphold our agreement with the FDA, we cannot claim these medicines diagnose, treat, cure, or prevent any specific illness or disease, nor are they appropriate as a complete substitute for professional care or a medical consultation. Please know herbal medicines ARE medicines and there may be interactions with your western pharmaceuticals. Please consult your pharmacist or prescribing doctor when deciding to use herbs and natural products.  These statements have not been evaluated by the FDA.
